COVID-19 Health Screening

The safety of our patients, staff and community is of utmost importance to us as we resume patient care at our practice.  To create the safest environment possible for everyone, we are implementing a COVID-19 health screening prior to anyone entering the office.

Temperature Screening

All staff, patients, and guests will have their body temperatures screened  with a contact-less thermometer prior to entering the office.  Anyone with a temperature of 100 degrees F or higher will be denied entry into the office.  Patients will be required to reschedule their visit until a later date.

COVID-19 Questionnaire

All staff, patients, and guests will be required to complete a COVID-19 Health Questionnaire prior to entering the office.  Please let us know AT LEAST 24 HOURS in advance of your appointment if any of the following apply to you as we will require you to postpone your appointment:

  • Your or anyone in your household has tested positive for COVID-19 within the last 30 days and has NOT had two negative test results 24 hours apart

  • You or anyone in your household has had a fever (100.4 F as per CDC) within the last 14 days

  • You or anyone in your household has had flu-like symptoms within the last 14 days

  • You or anyone in your household has flown on a plane in the last 14 days

  • You are currently experiencing two of the following symptoms or have experienced two of the following symptoms within the last 14 days:

    • Chills​

    • Shortness of breath

    • Muscle aches

    • Complete loss of smell or taste

    • Sore throat
